Depending on your budget, insurance, and issue, we have several counseling options to choose from. We’re ready to help you choose, or you can browse our counselor bios.
Once scheduled, you’ll receive a login to your private client portal. There, you will fill out new client paperwork and manage your appointment times and billing documents. Your clinician will review this paperwork prior to your first appointment.
**NOTE: Our office is open by appointment only as we do not have front desk staff.
New Client Paperwork:
We are a 100% paperless and virtual office. Once you’re booked, you’ll receive an email with paperwork to complete prior to your first appointment. However, if you feel more comfortable filling out paper intake forms, we will have them for you in our office when you arrive. They will be uploaded into your HIPAA compliant client portal, and then shredded (therefore, we highly recommend online forms).
**NOTE: If you do not fill out forms 24 hours prior to your appointment, your session will be canceled until you are able to complete them.
COVID Policy:
We adhere to updated CDC guidelines.
**NOTE: Some counselors in our office have individual needs with regards to masks and vaccines. Please ask our office manager or review their individual practice policy to be informed. Additionally, all counselors are prepared to wear masks if you request that they do so.
Fee Information:
SESSION FEES:
Session fees range from $45 to $165 depending on counselor experience. Our scheduling staff will be able to schedule you with a counselor who both meets your needs and meets your budget.
Legal fees: $200 per hour. This includes appearing in court, copying records, testifying, copying records, drive time, wait time, depositions, and court proceedings. new clients
FORMS OF PAYMENT:
Fees are due at the time of service. Our office takes Visa, MasterCard, American Express, Health Savings Accounts (HSA), Debit Cards, or PayPal (email to info@ichoosechange.com). All checks returned unpaid will be subject to a $25 administrative and processing fee.
**Please note that no-shows and cancellations not made 24 hours in advance by telephone will be billed at the full rate of your session. You are expected to make your appointment as arranged, and waiving this fee will be decided on a case-by-case situation. new clients
INSURANCE:
Some of our counselors accept BLUE CROSS BLUE SHIELD (BCBS), UNITED HEALTHCARE, AETNA, and CIGNA. Before scheduling, you will be given information that lets you know your client fee and insurance status.
We are out-of-network for all other plans and are happy to give you a superbill for services for you to submit for reimbursement with your provider. new clients

The Good Faith Estimate:
Every client who is private pay will receive a Good Faith Estimate.
The Good Faith Estimate shows the cost of items and services that are reasonably expected for your health care needs. The estimate is based on information known at the time the estimate was created. The Good Fath estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur. Federal law allows you to dispute (appeal) the bill if this happens.
This Good Faith Estimate is not a contract and therefore does not require you to obtain the items or services provided at I Choose Change. The client’s right to autonomy and self-determination is the foundation of a good therapeutic relationship between client and therapist. Therefore, you (as the client) have the right to terminate services at any time.
If you are billed more than the Good Faith Estimate, you have the right to dispute the bill.
You may contact your health care provider or facility listed to let them know the billed charges are higher than the Good Faith Estimate. You can ask them to update the bill to match the Good Faith Estimate, ask to negotiate the bill, or ask if there is financial assistance available.
You may also start a dispute resolution process with the US Department of Health and Human Services (HSS). If you choose to use the dispute resolution process, you must start the dispute process within 120 calendar days (about 4 months) of the date of the original bill.
There is a $25 fee to use the dispute process. If the agency reviewing your dispute agrees with you, you wi have to pay the price on this Good Faith Estimate. If the agency disagrees with you and agrees with your health care provider or facility, you will have to pay the higher amount.
